Position Summary
The Certified Nursing Assistant (CNA) – GIP Hospice Unit supports the delivery of compassionate, patient-centered care to terminally ill patients under the direction of a Hospice Nurse or Clinical Supervisor.
What You Will Need
Required Qualifications
- High School Diploma or equivalent
- Certified Nursing Assistant (CNA) designation
- CPR certification
- Strong interpersonal and communication skills
- Demonstrated ability to provide compassionate, ethical patient care aligned with Advocate Health values (Compassion, Excellence, Equity, Partnership, Stewardship)
Preferred Qualifications
- Completion of a CNA program
- Prior experience working with hospice or diverse patient populations
- Basic computer/keyboard skills
What You Will Do
Patient Admission, Transfer & Discharge Support
- Set up patient rooms and orient patients and families to the environment and equipment
- Obtain and document vital signs including height and weight
- Ensure all personal belongings are accounted for during transitions
- Support hospice philosophy including post-mortem care
Direct Patient Care
- Provide personal care including bathing, oral care, grooming, and skin care
- Assist with mobility, ambulation, positioning, and transfers
- Support toileting needs and apply skin protection measures
- Respond promptly to call lights and patient requests
- Assist with catheters, tubes, specimen collection, and bedside procedures
- Perform oral suctioning as delegated
- Assist with patient nutrition needs
Care Coordination & Documentation
- Document patient care in accordance with hospice and regulatory standards
- Communicate patient needs and status to the care team
- Support patient and family education
- Act as a patient/family advocate
- Maintain strict patient confidentiality (HIPAA compliance)
Unit Operations & Safety
- Maintain a safe patient environment and follow infection control protocols
- Respond to emergency situations appropriately
- Maintain equipment and cleanliness of patient care areas
- Participate in training, quality improvement, and competency development
- Provide 1:1 observation for high-risk patients when assigned
- Promote patient and family satisfaction
Physical Requirements & Working Conditions
- Ability to lift up to 35 pounds without assistance; use of lift equipment for heavier patients
- Exposure to clinical hazards including infectious diseases and medical equipment
- Ability to work nights, weekends, and holidays
- Ability to care for patients across all age groups

- Escalate concerns to prevent patient harm
