Job Description
- Provides oversight and direct supervision of the Community HealthWays program in two (or as assigned) LDH regions of the state.
- Supports and coaches CHWs on identifying health-related social needs, completing community outreach, and facilitating resource coordination.
- Monitors caseloads of all CHWs within their oversight by analyzing reports, monitoring client’s enrollment timeline, progress towards goals, and CHWs’ support of clients.
- Facilitates team meetings and other meetings as assigned to support the Community HealthWays Program in their regions of oversight.
- Meets, at minimum, bi-weekly with direct reports.
- Provides visibility to the Regional Leadership Teams in the development of community engagement and outreach plans.
- Serves as a bridge between the Office of Public Health’s Parish Health Units and the community by connecting clients to resources to improve their health and well-being.
- Leads in the identification and tracking of community resources, partners, and programs in their regions of oversight.
- Collects data and compiles reports for program evaluation.
- Conducts quality control review of their direct reports’ work.
- Conducts an annual performance evaluation for each direct report, setting future goals and strategies.
- Conducts recruitment, selection, training, supervision, and professional development of assigned staff.
- Other tasks as assigned.
QUALIFICATIONS
REQUIRED:
- Bachelor’s degree, or Associate’s degree plus 3 years of professional experience, or 6 years of professional work experience.
- Minimum 3 years professional experience within the healthcare, social services or community organization fields.
- Minimum 1 year professional supervisory experience.
- Excellent analytical and critical thinking skills; effective organizational and time management skills.
- Great attention to detail and follow up.
- Ability to manage projects, assignments, and competing priorities.
- Proficient in the use of Zoom, Teams, and Microsoft Office, including but not limited to Outlook, Word, and Excel.
DESIRED:
- Advanced degree.
- Minimum 4 years professional experience within the healthcare, social services or community organization fields.
- Minimum 3 years professional experience working with community programs and resources.
- Minimum 2 years professional supervisory experience.
- Relevant industry certifications.
SALARY: 55-65k
