Exciting opportunity to support both the Home Health and Hospital at Home teams.
This role will cover the Lake Zurich, Palatine, Arlington Heights, Des Plaines, and Park Ridge areas.
Full-time position with four 10-hour shifts per week, ideally from 10:00 a.m. to 8:00 p.m.
Every third weekend coverage is required.
Willing to train; strong clinical inpatient and/or home health skills are desired.

Pay Range
$35.50 - $53.25Major Responsibilities
- Complete scheduled patient visits and collaborate with the interdisciplinary team.
- Conduct comprehensive patient assessments and update plans of care as needed.
- Communicate treatment changes with physicians and the primary registered nurse.
- Monitor patient response to care and revise interventions accordingly.
- Coordinate scheduling and case conferences with primary nurse when appropriate.
- Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to ensure continuity of care after discharge.
- Communicates problems to physicians and primary Registered Nurse, and clearly states plans, actions and goals so that changes are reflected as they occur.
- Maintain accurate documentation in compliance with all regulations.
- Follow agency policies and safely perform required patient care activities.
- Provide age-appropriate care based on patient needs and developmental stage.
Minimum Job Requirements
Education
- Associate’s degree in nursing.
Certification / Registration / License
- Active RN license in state of practice.
- Basic Life Support (BLS) – American Heart Association
- Valid driver's license and reliable transportation.
Experience
- 3-4 years clinical experience (Med Surg, ICU or ER) AND
- 1-2 years in home health.

- Ability to work independently and make sound clinical decisions
- Excellent communication (written and verbal) and interpersonal skills.
- Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians.
- Cultural competence and respect for patient diversity.
- Good organization, prioritization and problem-solving skills.
Physical Requirements and Working Conditions
- Ability to multi-task with frequent interruptions.
- Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies
