POSITION SUMMARY
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Incorporates the six principles of trauma informed care throughout all aspects of their work
- Provide direct care and support to residents, including assistance with daily living activities, medication management, and crisis intervention.
- Participates in completing apartment inspections
- Notifies supervisor and/or maintenance of damages, need for repairs, and/or health and safety issues
- Collaborate with the treatment team to develop and implement individualized service plans for residents, ensuring their physical, emotional, and social needs are met.
- Facilitate group activities and programming to promote socialization, skill-building, and community integration.
- Maintain accurate and up-to-date documentation. Complete incident reports, adhering to confidentiality and privacy regulations.
- Participate in staff meetings, trainings, and supervision sessions to enhance knowledge and skills related to mental health and substance use disorders.
- Collaborate with external and internal agencies, such as case managers, therapists, or healthcare providers, to ensure coordinated and comprehensive care for residents.
- Adhere to all organizational policies, procedures, and ethical guidelines, including maintaining professional boundaries and confidentiality.
- Provides direct treatment according to agency policy and procedures.
- Completes other duties as assigned
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- High school diploma or equivalent; additional education or certification in a related field is preferred.
- Experience working with individuals with mental illness and substance use disorders, preferably in a residential or community-based setting.
- Knowledge of mental health and substance use disorders, including symptoms, treatment approaches, and recovery principles.
- Strong interpersonal and communication skills, with the ability to establish rapport and build trusting relationships with residents and colleagues.
- Ability to remain calm and composed in crisis situations, and effectively de-escalate conflicts or challenging behaviors.
- Demonstrated understanding of cultural diversity and the ability to work effectively with individuals from diverse backgrounds.
- Basic computer skills for documentation and communication purposes.
- Valid driver's license and reliable transportation, as occasional transportation of residents may be required.
- This position requires working in a residential facility, which may include evenings, weekends, and holidays.
