Licensed Vocational Nurse (LVN) – PM Shift (4-2 Rotation)

Yucaipa Hills Post Acute (formerly Calimesa Post-Acute) is an 82-bed skilled nursing facility seeking compassionate and dedicated LVNs for our PM shift. Our mission is to provide exceptional 24-hour care while supporting and valuing our staff.

Position Details
- Shift: PM (3:00 PM – 11:30 PM)
- Schedule: 4-2 Rotation
- Job Type: Full-time
- Pay: Based on experience

Key Responsibilities
- Administer medications and treatments per physician orders and facility protocols.
- Observe, document, and report patient responses to medications and treatments
- Perform patient assessments and contribute to care planning
- Maintain a safe, clean, and comfortable environment for patients and staff
- Follow infection control protocols and promote patient hygiene
- Implement and evaluate individualized care plans
- Assist with discharge planning, including treatments and physician orders
- Maintain a clean, safe, and comfortable environment for patients and staff
- Monitor and report changes in patient condition
Requirements
- Previous experience in a patient care setting preferred
- Strong understanding of the nursing process (assessment, planning, implementation, evaluation)
- Active LVN/LPN license in the State of California
- Current BLS (Basic Life Support) certification
- Commitment to precision, professionalism, teamwork, and high standards of clinical practice
