RN Patient Flow Coord, Call Center, Full Time, Days

7:00 AM - 7:00 PM. to include every other weekend. Responsibilities: Functions under the direction of the Utilization Documentation Manager and Director of Case Management. Facilitates access for community physicians/providers to physicians/providers at Self Regional Healthcare for the safe and efficient transfer and direct admissions of patients. Enhances and maintains constructive physician/caller/hospital/community relations. Acts as an intermediary between participating providers to achieve effective communication, working relationships and caller satisfaction. Demonstrates fiscal responsibility related to patient needs, resource utilization, payer requirements and organizational goals. Notifies registration of provided level of care per physician determination.
