Job Description
Essential Job Function
- Team Leadership: Provide leadership and guidance to team members, fostering a positive and productive work environment.
- Oversee the recruitment, training, and development of team members to ensure high performance and career growth.
- Monitor team performance, provide regular feedback, and conduct performance evaluations to recognize achievements and address areas needing improvement.
- Encourage and support the professional growth and development of team members through training, mentorship, and opportunities for advancement.
- Population Health Design: Oversight of population health teams which strive to improve community health care delivery, and the patient experience using clinical and financial performance information, proven methodologies, and successful partnerships.
- Design initiatives to improve the management of populations across the network that demonstrate measurable improvements in quality, service, and cost reduction.
- Continuous Improvement: Develop strategies for continuous improvement, implementing best practices and innovative approaches to enhance efficiency and effectiveness.
- Identify and address challenges and obstacles that arise, proactively seeking solutions to ensure the team's success.
- Coordinate the use of analytics, process improvement, etc to evaluate and improve the effectiveness of population health programs and interventions.
- Maintain responsibility for the growth, development and oversight of population health and clinical integration initiatives across the continuum of care.
- Stakeholder Management: Build and maintain relationships with key stakeholders, including clients, partners, and senior management, to ensure alignment and support for team objectives.
- Leads initiatives with cross-functional teams and senior leaders to achieve targeted strategic performance goals.
Education
- Required: Bachelor’s degree in nursing, Healthcare Administration, Public Health, Pharmacy, or related field.
Experience
- Required: 5 years of experience in healthcare management, with a focus on population health management or care coordination. 3 years’ experience in value-based care, ACO’s, or clinically integrated networks.
