Social Worker - Discharge Planner

  What You'll Do:   As a Social Worker, you will advocate for the well-being of our residents and ensure their holistic needs are met.  Your work will play a pivotal role in enhancing the quality of life of our residents.  You will also be responsible for discharge planning, helping to coordinate safe, smooth, and well-supported transitions of care. This includes collaborating closely with residents, families, and the interdisciplinary team to ensure continuity and appropriate post-discharge resources.
