Description

• Coordinate and assess patient care plans and ensure seamless delivery of healthcare services.
• Monitor patient progress and record medical data accurately.
• Administer prescribed medications and treatments.
• Educate patients and their families on post-operative care and health maintenance.
• Collaborate with healthcare professionals to optimize patient outcomes.

• Active Registered Nurse (RN) license.
• Minimum of 3 years of experience in a clinical setting.
• Strong understanding of medical practices and patient care procedures.

• Experience in care coordination is a plus.
• Bachelor's degree in Nursing (BSN) preferred.

• Associate's or Bachelor's degree in Nursing.
• Current RN License.

• Exceptional communication and interpersonal skills.
• Strong critical thinking and problem-solving abilities.
• Ability to work independently and as part of a team.
