How you’ll contribute:
- Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
- Creates and maintains up to date aftercare coordination community resources
- Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.
- Provides resource information to patients and families as appropriate for future care needs.
- Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.
- Provides resource information to patients and families as appropriate for future care needs.
- Creates a detailed Discharge Summary and Contract for each patient as assigned.
- Completes all hospital and client documentation thoroughly, timely, and in accordance with hospital standards.
- Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs, assisting others to ensure patient safety.
- Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards.
- Demonstrates a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws.

What we’re looking for
Applicants should possess:
- Education: Bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
- Experience: Prior experience with psychiatric and chemical dependency patients.
- License: Unencumbered clinical license if eligible/needed per state regulations preferred. CPR certification and Crisis Prevention Training (CPI) within 30 days.
