Job Description
SUMMARY
The Registered Nurse (RN) provides direct nursing and healthcare services under the direction of the Medical Director and in accordance with the PPSE Medical Standards and Guidelines. The Centralized Follow-up Coordinator role encompasses the oversight, coordination, and direct participation in PPSE’s centralized follow up program. The RN/Centralized Follow-up Coordinator is a member of the healthcare team and is expected to keep a current base of knowledge on abortion, contraception, STIs, surgical nursing support, nursing best practices, and current trends in sexual and reproductive healthcare and to participate in PPSE’s after-hours call rotation.
ESSENTIAL RESPONSIBILITIES
General
-
Accurately documents all care, services, diagnoses, and treatments properly in patient medical records
-
Answers client calls (including participation in the after-hours call rotation) and gives client assistance in regards to referrals, medical questions, and general information
-
Completes follow-up responsibilities and participates in PPSE’s centralized follow-up program
-
Triages clients and helps in maintaining clinic flow
-
Dispenses contraceptives and medications according to protocols
-
Completes nursing duties in a timely manner
-
Assist with the opening and closing of clinic as assigned
-
Maintains and protects client confidentiality at all times
-
Ensures compliance with all applicable laws and regulations governing the clinic
Patient Counseling and Education
-
Provides counseling and patient education on contraception, medication, medical treatments, lab tests, medical recommendations, and/or referrals
-
Provides pre-abortion counseling to patients, including a copy of the printed materials developed by the Georgia Department of Public Health
-
Provides instructions and counseling to recovery room patients
Surgical Care
-
Provides all nursing care of patients in preparation for procedures, during procedures, the recovery period, and discharge by the physician
-
Assists Physicians and Nurse Practitioners and provides care as directed
-
Takes responsibility for the crash cart remaining stocked, open and available on surgical days
-
As authorized, manage and monitor analgesia and sedation during therapeutic, diagnostic, or surgical procedures to include administration of IV medication and documentation and monitoring of vitals signss (e.g. oxygen saturation, blood pressure, and pulse)

Supports and participates in organizational planning to continuously improve patient outcomes
-
Assists in other areas of the clinic as needed and as directed by physician and/or Health Center Manager (i.e. medical records, front office, etc.)

Participates in referrals, follow up and quality control programs
-
Identifies opportunities to improve clinic work processes and partners with team members to implement
Lab
-
Assists in the lab and exam room as needed
-
Assist providers with lab follow-up and required state reporting
Centralized Follow Up Coordination
-
Acts as a resource for licensed and non-licensed members of the PPSE’s healthcare team participating in centralized follow-up, ensuring work performed is quality driven and compliant with all participating regulating entities
-
Responsible for training licensed and non-licensed staff on PPSE’s follow up policies, procedures, and processes
-
Manages centralized follow up and after-hours call scheduling
-
Works with the PPSE Medical Management Team on quality and process improvement initiatives and serves as a standing member on the PPSE Clinical Operations Subcommittee
-
Participates in auditing charts to assure compliance with MS&Gs and state regulations
-
Be forthcoming and truthful in responding to the Alabama Department of Public Health, Georgia
Department of Community Health, and the Mississippi State Department of Health inquiries and
request for information
-
Identifies opportunities to improve clinic work processes and partners with team members to implement
-
Supports and participates in organizational planning to continuously improve systems/processes to ensure we are providing quality care and exceptional customer service and improve patient outcomes
Follow-up
-
Participates in centralized follow-up for all PPSE sites; referrals, follow up and quality control programs
-
Explains abnormal lab result findings to patients including, the nature of the abnormal findings, the implications of the findings, the possible consequences of not receiving additional diagnosis and/or treatment, an explanation of management options and provision of appropriate referral sources, as indicated, informing client that it is their responsibility to obtain follow-up care
-
Generates follow up reports and carries out telephone, mailed or electronic notifications per PPSE protocol
-
Provides notification to patients for abnormal lab and imaging results as assessed by clinicians
-
Consults with clinician as necessary for medical related calls from patients that are beyond their scope of knowledge/practice.
-
Identifies appropriate referral cases based on information collected from physicians, members of the health care team, review of the medical record and use of high risk screening criteria
-
Sends Georgia/Alabama/Mississippi Department of Health, Practitioner Disease Report Form for all positive HIV, GC, CT and RPR to appropriate County Health Department and signs off on normal results
-
Maintain up-to-date and accurate handouts, notification letter, reminder letters to mail to patients for all referrals and follow-up
-
Organizes and send appropriate notification letter and fact sheets for first, second and third contact attempts.

Sends reminder letters within specified time frame for required labs.
-
Makes appointments in EPM for patients when necessary.
-
Acts as an integral player in the development of high-risk case management to include items such as beta quantitative HCG follow-up, PAP follow-up
-
Communicates effectively with medical leadership in regard to follow-up plans, using the PPSE Standards and Guidelines as a resource
Additional Responsibilities

Be forthcoming and truthful in responding to The Georgia Department of Public Health inquiries and requests for information
