Mass General Brigham Medical Group is seeking an experienced and highly independent Lead Clinical Social Worker to provide virtual support across our primary care practices throughout Massachusetts, New Hampshire, and Maine.
This unique float role serves as a critical resource to support practice coverage during leaves of absence, vacations, staffing transitions, and periods of increased patient volume. Working entirely remotely, the Lead Clinical Social Worker Float will provide direct patient care, collaborate with interdisciplinary teams, and proactively identify areas where social work support is needed.
The ideal candidate is a self-starter who thrives in a dynamic environment, can quickly adapt to different practice settings, and is comfortable working independently while supporting patients and care teams virtually.
Schedule
· Full-time, Monday through Friday from 8:30am-5:00om
· Fully remote/virtual position
· No travel required
Key Responsibilities
Responsibilities include, but are not limited to:
· Provide comprehensive virtual social work services to patients within family medicine and internal medicine practices across the Medical Group.
· Conduct psychosocial assessments and develop appropriate intervention and care plans.
· Support patients and families by connecting them with community resources, behavioral health services, and other support programs.
· Provide coverage for social work staff during leaves of absence, vacations, staffing gaps, and other operational needs.
· Collaborate with physicians, advanced practice providers, nurses, care coordinators, and other interdisciplinary team members to ensure coordinated patient care.
· Independently manage patient caseloads and prioritize work in a fast-paced, virtual environment.
· Proactively identify areas of high patient volume and emerging team needs, stepping in to provide support with minimal direction.
· Serve as a clinical resource and support partner to care teams across multiple practices.
· Maintain timely and accurate documentation in accordance with organizational, state, and federal requirements.
· Participate in quality improvement initiatives and contribute to the ongoing development of social work services within the Medical Group.

Qualifications
Who We’re Looking For
✔ Master’s Degree in Social Work (MSW) or Mental Health.
✔ LICSW or LMHC licensure in Massachusetts.
✔ Minimum 5 years of experience in a clinical or healthcare setting, with supervisory experience preferred.
✔ Strong knowledge of therapeutic interventions, crisis assessment, and case management.
✔ Excellent interpersonal, leadership, and communication skills.
✔ Ability to work independently and collaboratively in a fast-paced environment.
