Job Description

Overview
Social Worker – Case Management in Everett, WA. This travel assignment begins 06/29/2026 for 13 weeks on a 4x10 Days shift. Weekly pay ranges from $1,720 to $1,800, with 72 hours guaranteed per 2-week period. Orientation is 40 hours. The role supports inpatient adult and geriatric medical social work and case management in an acute care setting.

Responsibilities
- Provide psychosocial assessment and care planning for adult and geriatric inpatients; develop and implement discharge plans.
- Coordinate care transitions, including referrals to rehabilitation, long-term care, and community resources; facilitate guardianship and advance directives as appropriate.
- Collaborate with an interdisciplinary team (e.g., physical therapy, pharmacy, interpretation services, social services, transportation) to execute comprehensive care plans.
- Document assessments, plans, and progress in the designated electronic charting system; maintain accurate caseload management and participate in rounds.
- Manage a caseload with attention to risk factors, patient advocacy, and resource utilization; respond to referrals in a timely manner and adjust plans as needed.
- Maintain compliance with relevant regulations (Medicare/Medicaid) and support needs assessment, guardianship, and eligibility processes.

Requirements
- Licensure: Active Washington state license in hand for the clinician. RN licensure or WA social work licensure (LICSW, LSWAIC, LASW, LSWAA) with WA license in hand accepted.
- Certifications: BLS certificate.
- Experience: Minimum 1 year of clinical experience; acute care experience with discharge planning / care coordination required; diverse case management experience preferred (Med-Surg, ED, etc.).
- Education: RN (BSN) or MSW required.
- Charting: EPIC experience required.
- Workplace readiness: Ability to work in an inpatient setting; weekend and holiday coverage as scheduled; ability to float to related units and campuses as needed.
