Position Summary
This role focuses on providing intensive case management services to youth (ages 18-24) experiencing homelessness, helping them secure and maintain stable housing. The Case Manager will develop individualized service plans, coordinate with landlords, connect clients with resources, and provide ongoing support to ensure housing stability. The Case Manager will also collaborate with team members and community partners to ensure program compliance and the success of the Rapid Rehousing and Transitional Housing programs.
Key Responsibilities
Case Management and Client Support
- Provide intensive case management services, including ongoing support, crisis intervention, and life coaching.
- Conduct client intake and assessments, ensuring eligibility for the Rapid Rehousing program and completing required documentation (e.g., HMIS).
- Develop Individual Success Plans (ISP) with clients upon assessment completion, revising and updating plans weekly to reflect progress and goals.
- Assist clients in locating housing, completing applications, and understanding lease agreements.
- Provide mediation and advocacy with landlords and neighbors to address housing-related issues.
- Facilitate referrals for employment, education, mental health support, financial assistance, and other services.
- Conduct home visits to monitor client progress, ensure housing stability, and support goal achievement.
- Support clients in transitional housing by offering life skills coaching, goal setting, and access to resources.
- Participate in regular case conferencing sessions with team members and community partners to review client progress, address challenges, and develop collaborative strategies for achieving housing stability and program goals.
Housing Navigation and Landlord Relations
- Identify and secure housing options in the private rental market that meet client needs.
- Build and maintain strong relationships with landlords, property managers, and community organizations.
- Educate landlords on program requirements and provide support to address potential issues.
- Conduct habitability and lead-based paint inspections to ensure housing compliance.
Program Compliance and Documentation
- Maintain accurate and confidential client records in compliance with program and regulatory standards.
- Document client interactions, progress notes, and outcomes in systems such as HMIS and EHR.
- Monitor program outputs and ensure all required documentation is completed for external grants and reporting.
- Prepare and present cases during weekly case review meetings and facilitate Client Success Planning Meetings.
- Document critical incidents, follow-up actions, and other pertinent information as necessary.
Group Facilitation and Life Skills Development
- Develop and facilitate life-skills workshops, well-being sessions, and support groups for program participants.
- Engage clients in positive transition planning and goal-setting to promote self-sufficiency.
- Facilitate Success Planning Meetings with clients and their identified circle of support.
Professional Engagement and Collaboration
- Actively participate in team meetings, case conferences, and training sessions to enhance service delivery.
- Collaborate with community partners, service providers, and faith-based organizations to connect clients with resources.
- Attend housing-related community events and represent the organization in collaborative efforts to address homelessness.
Professionalism
- Maintain punctuality and consistency for assigned shifts, meetings, and community events.
- Perform duties as assigned to meet the needs of clients and ensure adherence to program expectations, including facilitating success plan meetings, ensuring apartment cleanliness and safety, and providing crisis intervention as necessary.
- Attend all required trainings, including agency-provided and external seminars or conferences related to working with youth.
- Other program duties as assigned by the Director of Behavioral Health.
Additional Requirements
- Reliable Transportation: Must have a reliable vehicle, a valid Nevada driver’s license, and updated insurance.
- Caseload Management: Must be able to manage a caseload of 15 clients.
- Home Visits: Must be available for home visits and to see clients at the agency or in their homes.
- Flexibility: Must be willing to work occasional evenings and weekends based on program needs, especially for IOP services.
Skills and Qualifications
- Proficiency in case management, including developing individualized plans and providing ongoing support.
- Strong knowledge of the Housing First model, harm reduction principles, and trauma-informed care.
- Expertise in housing navigation, assisting clients with securing housing and understanding lease agreements.
- Strong advocacy and mediation skills to resolve conflicts between clients and landlords or other stakeholders.
- Knowledge and application of trauma-informed care principles to effectively support clients with complex needs.
- Experience implementing harm reduction strategies to promote client stability and autonomy.
- Cultural competency with the ability to work inclusively and respectfully with diverse populations.
- Excellent interpersonal communication skills to build trust and maintain positive relationships with clients and partners.
- Proven problem-solving abilities to address client challenges and develop effective solutions.
- Critical thinking skills to adapt to changing client needs and make informed decisions.
- Strong organizational skills to manage multiple tasks, deadlines, and client records efficiently.
- Proficiency in Microsoft Office, Google Workspace, and data management systems (e.g., HMIS).
- Conflict resolution expertise to de-escalate tense situations and mediate disputes effectively.
Education and Work Experience
- Preferred: Lived experience with homelessness, housing instability, or navigating social services is highly valued.
- Required:
- 2+ years of case management experience, preferably working with youth experiencing homelessness.
- OR Associate’s degree in a related field with 2 years of case management experience.
- Experience supporting individuals in transitional or rapid rehousing programs is strongly preferred.
Physical Requirements
- Flexibility to work occasional evenings and weekends based on program needs.
- Reliable transportation required for meetings, client visits, or other professional commitments.
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at a time.
- May be required to work outdoors in extreme weather conditions or indoors with loud noises
Starting pay: $26.44 per hour
