Position Overview
Vasona Creek Healthcare is seeking a dedicated and compassionate Case Manager RN to join our full-time care team. This role is ideal for a licensed vocational nurse who excels in care coordination, interdisciplinary collaboration, and patient advocacy within a skilled nursing or healthcare setting.
Key Responsibilities
Coordinate and manage patient care plans from admission through discharge
Collaborate with physicians, RNs, therapists, social services, and interdisciplinary teams
Monitor patient progress, treatments, and outcomes to ensure quality care
Communicate effectively with patients, families, and healthcare providers
Ensure compliance with facility policies, regulatory requirements, and documentation standards
Assist with utilization review and discharge planning as needed
Qualifications
Current RN license in good standing
Prior case management, skilled nursing, or long-term care experience preferred
Strong organizational, communication, and documentation skills
Ability to work collaboratively in a fast-paced healthcare environment
Proficiency with electronic medical records (EMR) systems preferred
Compensation & Benefits
$50-56 salary
