Compensation: $47.00 - $50.00 / hour

If you’re a Physical Therapist (PT) who wants to build meaningful relationships with your patients and see the long-term impact of your care—this is it.
In this setting, you will work with Medicare Part B patients in Assisted Living and Independent Communities, allowing you to follow patient progress over time and focus on improving independence, mobility and quality of life.
Unlike fast-paced short-stay environments, this role allows you to provide purposeful, patient-centered therapy, with consistency and clinical autonomy.

Key Responsibilities
- Evaluate and treat residents in Assisted Living & Independent Living Communities
- Develop personalized care plans focused on mobility, safety, and independence
- Deliver care through therapeutic exercise, functional mobility, balance activities, and manual techniques
- Help patients improve strength, balance, and overall quality of life
- Collaborate with care teams, staff, and caregivers to achieve strong patient outcomes
- Document patient progress and adjust treatment plans as needed
Skills, Knowledge and Expertise
- Licensed Physical Therapist (PT) or license eligible in the state of practice
- Graduate of an accredited Physical Therapy Program
- Interest or experience in geriatric rehabilitation or senior living therapy
- Passion for patient-centered care and improving quality of life for older adults
