
Executive Director, Hospital at Home Clinical Care and Operations
Job Description
The Executive Director, Hospital at Home (HaH) Clinical Care and Operations provides executive leadership for the operational strategy, implementation, and performance of Atlantic Health’s Hospital at Home program. This role is responsible for building and scaling a high-performing integrated care delivery model that enables safe, high-quality acute care in the home setting.
Reporting to the Vice President, Healthcare at Home and Post-Acute Services and partnering closely with the Chief Medical Officer, Hospital at Home, this leader oversees nursing operations, clinical operations, logistics, virtual care, workforce operations, vendor partnerships, and program administration across the HaH organization.
The Executive Director is accountable for operational excellence, patient and caregiver experience, regulatory compliance, workforce engagement, and financial performance. This leader will remain at the forefront of emerging models and best practices in home-based acute care delivery and serve as a national thought leader for HaH advancement. The Executive Director will serve as a key enterprise advocate for Hospital at Home innovation and best practices, helping expand access to care, improve patient outcomes and experience, optimize hospital capacity, and provide cost-effective alternatives to traditional inpatient care.
PRINCIPAL RESPONSIBILITIES:
1. Strategic Leadership and Growth
• Partner with executive and clinical leadership to develop and execute the strategic growth plan for Hospital at Home services.
• Identify and advance opportunities for program expansion, geographic growth, payer alignment, and operational scalability.
• Support enterprise strategies related to quality, experience, workforce engagement, growth, and financial sustainability.
• Serve as a leading voice – across the enterprise and externally – in advancing Hospital at Home operational innovation and industry partnerships.
• Monitor market trends, regulatory developments, and emerging operational best practices to strengthen program performance and positioning.
2. Program Design, Build and Implementation
• Oversee the design and development of the operational infrastructure and workflows required to support Hospital at Home services, including logistics, supply chain, virtual care operations, workforce deployment, and enabling technologies.
• Establish operational standards, service-level expectations, and vendor performance frameworks supporting acute care delivery in the home.
• Lead operational implementation and scaling of HaH capabilities across the enterprise.
• Partner with enterprise operational, digital, legal, regulatory, sourcing, and finance teams to support HaH performance, growth and sustainability.
3. Operational, Clinical, and Performance Leadership
• Provide executive oversight for HaH nursing, operational, and administrative functions, including intake, field operations, virtual care, and remote patient monitoring services.
• Lead operational integration between HaH and enterprise clinical and hospital-based services.
• Develop and support effective, efficient internal controls to ensure that policies and operating procedures are followed.
• Oversee workforce planning, operational workflows, resource deployment, and capacity management across the HaH organization.
• Develop high-performing teams and foster a culture of collaboration, accountability, innovation, and service excellence.
• Oversee the HaH virtual command center and related operational support functions.
• Lead continuous improvement initiatives focused on operational effectiveness, service quality, regulatory readiness, and scalability.
• Analyze operational workflows and procedures and identify and implement continuous improvement opportunities.
• Ensure that the voice of the patient – and their caregiver – is represented and persists throughout HaH care delivery and services.
4. Financial Management
• Partner with finance and executive leadership to develop and manage operating and capital budgets for the HaH organization.
• Monitor operational and financial variances and performance metrics and implement corrective actions as needed.
• Support forecasting, resource planning, and financial optimization strategies aligned with organizational goals.
5. Stakeholder Engagement
• Serve as a key operational leader and change agent in advancing HaH adoption across the enterprise.
• Build strong partnerships across clinical, operation, and administrative teams to support successful program integration and growth.
• Promote alignment, engagement, and communication among internal and external stakeholders supporting HaH services.
Salary range: $243,594 - $455,500 base only; excludes all incentives.
QUALIFICATIONS
• Bachelor’s degree in Nursing required; Master’s degree in Business, Healthcare Administration, Public Health, or related field preferred
• 7+ years of progressive healthcare leadership experience, including operational and clinical management responsibilities
• Demonstrated experience managing operational, quality, and financial performance in a healthcare environment
PREFERRED EXPERIENCE
• Experience leading complex, matrixed healthcare operations and cross-functional initiatives
• Experience developing or scaling innovative care delivery models
• Demonstrated experience leading transformational change initiatives - a change agent
