Case Manager (CM) PRN

Oversee half of the census-can range from 10-15. Case manage, answer patient/family questions, See each patient 2x week minimum, order DME, track team conference meeting requirements, complete spreadsheets, make/cancel outside appointments. MUST be willing to push hard to get patients home and work through barriers. Work closely with nursing and therapy team to determine barriers to discharge and encourage patient and family to prep for home discharges. Typically M-F 8-5
