Join our team as a day shift, full-time, Population Health LPN (Licensed Practical Nurse) Patient Care Coordinator in Tulsa, OK.

  • Utica Park Clinic, founded in 1982, is a multi-specialty medical group with more than 300 employed physicians and advance practice providers representing more than 30 specialties across 82 sites in Oklahoma, serving 21 communities.

The care coordination process includes outreach to patients, generally outside of face-to-face office visits, to help them understand and manage their health, navigate the health system, and meet their health goals. This position requires expertise in the practical nursing process including the ability to provide focused assessment of acute and chronic conditions, disease management education, and coordination of care across the healthcare continuum.

Job Requirements:

  • Graduate of an accredited school of nursing
  • Possession of a valid State LPN license
  • BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care

Preferred Job Requirements:

  • A minimum of two years of professional nursing experience in a clinic setting
