Understands the necessity for timely completion of medical record documentation by analyzing medical records for missing documentation and signatures. Compares the documentation in the medical record against required standards and enters deficiencies for providers in the electronic medical record system.May be assigned other HIM functions to support departmental workflows.
Job Description
Performs chart analysis and/or abstracting on a variety of chart type and recognizes the relation of a complete medical record and enters accurate deficiencies. Recognizes the importance of their work to the provider suspension program. Contributes to the medical staff timeliness and suspension regulations through accurate notation and monitoring of deficiencies in the medical record. Utilizes job specific software in analysis and monitoring functions. Extensive use of the electronic health record. Applies knowledge of disease processes, anatomy, physiology, medical terminology, Joint Commission (JCAHO), state laws, and other regulatory standards in the analysis of the medical record. May perform birth certificate workflow to support new births obtaining a legal name in a timely manner. When performing birth certificate workflows, performs work in a timely and accurate manner. Uses State software to submit baby's legal name and updates baby's name in Epic. Performs paternity paperwork in an accurate manner and mails documentation timely. Maintains good working relationships with nursing teams and handles challenging family situations in a professional manner. May coordinate annual review of provider orders/order sets and aligns downtime forms. Assist with the development and management of forms scanned into the patient medical record.Qualifications
Associate's degree in Health Information or Medical Record Technology required. In lieu of education requirement, may consider the equivalent of two years of Health Information Management (HIM) or revenue cycle experience.One year of health information or revenue cycle experience in health care required.
Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) preferred.
