Citrus Heights Post Acute is seeking a compassionate and driven Speech-Language Pathologist (SLP) to join our skilled nursing team. In this role, youβll help patients improve communication, swallowing, and cognitive function while working alongside a collaborative interdisciplinary team.

Key Responsibilities
- Evaluate patients within 24 hours (or 1 business day) of physician referral
- Develop and implement individualized treatment plans
- Provide speech therapy services in accordance with physician orders
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate, timely documentation in EMR systems
- Collaborate with physicians, nursing staff, and rehab team members
- Participate in care conferences, rehab meetings, and discharge planning
- Educate patients, families, and caregivers on home programs and ongoing care
- Provide staff training and in-services as needed
Qualifications
- Masterβs or Doctorate degree in Speech-Language Pathology
- Active California SLP license (required)
- Strong clinical, communication, and documentation skills
- Skilled nursing or post-acute experience preferred (not required)
- Ability to thrive in a fast-paced, team-oriented environment
Certifications & Requirements
- Current SLP license in good standing
- Ongoing continuing education per state requirements
- Experience with or willingness to learn EMR systems
