Job Description
POSITION SUMMARY: This position of Physical Therapist Assistant (PTA) is responsible for delivery of physical therapy treatment to PACE participants under the direction of a supervising Physical Therapist. The PTA will provide therapy to participants per the established treatment plan, and within the PTA scope of practice. Therapy will be provided to Rocky Mountain Health Care Services (RMHCS) program participants in a variety of settings, including outpatient, home, assisted living facilities, and long-term care facilities.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Provide physical therapy treatment, under the direction of a supervising Physical Therapist across several practice locations including outpatient, home, assisted living facilities, and long-term care facilities
- Trains family members, caregivers, and other staff members in methods to enhance mobility, safety, and functional independence of participants
- Collaborates with supervising PT and other team members on participant care issues, treatment plan progression, and participant functional status
- Monitors participant/therapy schedules and physical therapy orders to ensure timely provision of ordered physical therapy services
- Provides input into departmental quality improvement efforts
- Assists with the coordination of care of participants as an active member of the interdisciplinary team
- Other duties as requested/assigned
- Graduate of an accredited physical therapist assistant program is required
- PTA Licensure in the State of Colorado is required
- One year of general physical therapy assistant experience in a geriatric setting is preferred
- Knowledge of how to use and maintain physical therapy equipment
- Ability to work independently with minimal supervision
- Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
- Must have working knowledge of computers and Microsoft Office applications
