Registered Nurse - Outpatient- Forensic Assertive Community Treatment (FACT)

The FACT Registered Nurse is an active member of the care team and provides high-quality skilled nursing services in the community, including patient education about mental illness diagnosis and symptoms, independent living skills, and illness management. FACT nurses also provide crisis stabilization and prevention, as well as information and/or referral for community resources.
