Your Clinical Impact:

  • Evaluation & Planning: Conduct comprehensive resident evaluations and create evidence-based treatment plans tailored to individual recovery goals.

  • Direct Treatment: Provide high-quality, hands-on therapy in line with the California State Practice Act.

  • Clinical Leadership: Supervise Physical Therapist Assistants (PTAs) and guide Restorative Aides to ensure consistent care delivery.

  • Precise Documentation: Maintain accurate daily notes, progress reports, and recertification documentation in a timely manner.

  • Interdisciplinary Collaboration: Collaborate closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.

  • Discharge Success: Educate residents and families on safe mobility and home-exercise programs; recommend and coordinate necessary durable medical equipment (DME) such as walkers and braces.

Qualifications:

  • Licensure: Valid, unrestricted California Physical Therapist (PT) License.

  • Skills: Strong clinical reasoning, accurate documentation, and the ability to motivate residents throughout their recovery journey.

  • Relationship Building: A positive attitude with a talent for building rapport with residents, families, and professional colleagues.
