Your primary responsibilities will include coordinating the Individual Support Plan (ISP) development, ensuring team members are trained, and conducting evaluations to maintain high standards. You will also oversee documentation and monitor the well-being of individuals, ensuring a safe and clean environment.

Key Responsibilities:
· Maintain adherence to System Policies and Procedures and department-specific procedures.
· Develop and monitor the Individual Support Plan (ISP) with measurable outcomes.
· Train team members in their responsibilities according to the ISP.
· Document services and ensure compliance with HIPAA regulations.
· Supervise and provide evaluations for team members.
· Conduct quarterly Record and Therapy Reviews to validate documentation.
· Make weekly visits to the ISL for health and safety monitoring.
· Maintain client profiles in the database and prepare annual budgets.
· Track monthly exceptions and prepare variance reports.
· Collaborate with support staff and the residential nurse for medical care.
· Facilitate community participation and opportunities for natural support.
· Perform additional duties as assigned.

Education, Experience, and/or Credential Qualifications:
· A bachelor’s degree from an accredited college or university.
· Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree.

Additional Qualifications:
· Must have a reliable means of communication.
· Proficient in reading and understanding written materials.
· Ability to write clearly and legibly.
· May be required to assist with physical transfers or lifting based on individual needs.

Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or negligible force frequently (exists 1/3 to 2/3 of the time). Repetitive movements for typing and/or writing as needed. Sedentary work involves sitting most of the time but can include occasional walking or standing.
