RN - Oncology - Patient Nurse Navigator

The Nurse Navigator enhances the patient experience by providing a seamless navigation process from abnormal finding/diagnosis through the cancer continuum.  Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families.  Works with patients and families to answer questions, provide emotional support, and resolve any barriers, needs or issues related to the cycle of cancer care.  Assesses patients’ current and future needs, provides patient access to resources internal and external, facilitates distress...
