Healthcare Coordination Specialist

Pay: $21-22/hour. The Healthcare Coordination Specialist will develop strategies to link residents with health care and care coordination services. You will build relationships with primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position that will be performed onsite.
