The Lifestyle Coach, Diabetes Prevention Program is a key member of the Community Health and Well-Being Department at St. Mary’s Health Care System. This role leads participants across our three hospital systems through a year-long lifestyle change program designed to prevent or delay the onset of type 2 diabetes. Lifestyle Coaches empower participants to make sustainable behavior changes by providing education, guidance, and support in developing healthy eating habits, increasing physical activity, and maintaining long-term wellness goals.Fulltime position- various hours (Days, evenings and weekends- based on the various class schedules)
What you will do:
- Facilitate sessions over the course of the 1-year program (weekly sessions for 16-26 weeks followed by monthly sessions for the remainder of the year)
- Continually reinforce and integrate session material, build on concepts by referring back to previous sessions and discussions
- Support participants by providing information, encouraging progress, and working with the group to address challenges or barriers that arise
What you will need:
- Education: High School Diploma required. Bachelor’s degree in public health/ health Promotion is preferred.
- Certifications: Medical Assistant Certification is preferred.
- Experience: Must have at least 1-year prior clinical experience in a Hospital or Doctors office setting to be considered for this position.
- Must be able to work across all three hospital systems and facilitate classes at multiple locations.
