Care Manager RN Per Diem Variable

Care Manager RN at St Joseph Hospital in Eureka, CA. This position is Per Diem and will work 8-hour, Variable shifts. The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates on a daily basis with medical staff, hospital personnel and outside resources to ensure the appropriate utilization of resources according to clinical and financial standards. Monitors the medical record for variances to clinical and financial standards and intervenes to achieve the optimum outcome for the patient and the hospital.
