Responsibilities
Works under the supervision of physician on-duty (CCMC) or pharmacist on duty (CRMC/CCMC) to support medication reconciliation by improving the accuracy of the patient's medication history which will lead to improved patient safety and satisfaction. Obtains accurate medication history by collecting medication record information from multiple sources and entering current patient medications into the hospital's electronic medical record for utilization by the nurses and physicians responsible for admitting and discharging patients from the hospital.
Qualifications
Education
- High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required.
Licenses
- RXTECH - State of California Pharmacy Technician License required
