Care Team Specialist - Zion Care Link (rural site - eligible for stipend) - Iowa CCBHC

Serving clients across rural communities in Southwest Iowa, the Care Team Specialist plays a key role in improving access to care in areas where services are limited and relationships matter. The position works closely with Zion staff and Heartland Family Service to strengthen coordination, improve data sharing, and support continuity of care. This role operates within a flexible work environment that values collaboration, adaptability, and responsiveness to client and community needs while strengthening the rural mental health care continuum.

Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience). Work Schedule: 37.5 hours per week (Monday through Thursday, 8:30 a.m. to 5:00 p.m.; Fridays 8:30 a.m. to 2:30 p.m.)
