Your role involves collaborating with healthcare professionals and others as needed to address our clients’ medical, social, and emotional needs of our clients, ensuring they receive the highest quality of care. Community Case Manager: Mainly works with clients who go to non-Vivent Health Clinics for HIV care. Main coverage for Case Manager on Duty.
