RN Liaison

Reporting to the Manager of RN Liaisons, the RN Liaison facilitates referrals to hospice, assists indeveloping relationships with business partners in the community, and performs timely, individualized, accurate assessmentsfor patients referred to the organization. The RN will represent GHS in a professional manner and is committed to delivering the highest quality of care possible as identified in the GHS mission statement.
