Schedule: Full Time
Shift: Day
Location: Field work – covering Adams, Arapahoe, Jefferson, Denver, Douglas, Elbert and Elizabeth Counties.
- Provides hospice nursing services to patients at home or in nursing facilities.
- Initiates and manages plans of care for an assigned caseload of patients.
- Coordinates and oversees patient care performed by Certified Nursing Assistants.
- Conducts patient assessments and provides skilled nursing care.
- Makes recommendations and collaborates with physicians.
- Utilizes clinical nursing skills to manage patient caseload per hospice policies and regulatory standards.
- Provides comfort and support to family members and caregivers.
- Educates families, caregivers, and patients on end-stage disease processes, safety issues, and symptom management.
- Participates in interdisciplinary team meetings.
- Orders necessary resources and supplies, including medications, supplies, and medical equipment.
- Completes all required documentation in the electronic medical records system.
- Serves on-call on with other team members for team emergency clinical needs after-hours.
- Other duties as assigned.
Knowledge, Skills, and Abilities:
- Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
- Knowledge of electronic medical record. [Required]
- Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
- Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
- Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures, and other instructions furnished in written, oral, diagram, or schedule form. [Required]
- Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
- Ability to work independently and manage multiple tasks with prioritization of their importance. [Required]
- Demonstrates critical thinking and problem-solving skills. [Required]
- Demonstrates effective organizational and customer service skills. [Required]
- Ability to work and communicate with people of all social, economic, and cultural backgrounds; be flexible, open-minded, and adaptable to change. [Required]
- Knowledge of assessment competency and application for all ages served within the long-term acute care environment. [Required]
- Demonstrates leadership skills and the ability to innovate and identify discharge options for medically complex patients. [Required]
- Knowledge of the insurance industry and government reimbursement for healthcare settings. [Required]
- Flexible in a complex and changing healthcare environment. [Required]
- Computer proficiency with Outlook email and electronic medical records. [Required]
- Knowledge of community resources and post-acute care programs across the continuum. [Preferred]
- Understanding of CMS Conditions of Participation for Discharge Planning. [Preferred]
- Demonstrates conflict management and resolution skills. [Preferred]
- Embraces teamwork principles. [Preferred]

- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
Field of Study:
- N/A
Work Experience:
- 2+ of hospital nursing experience [Preferred]
- Specific experience in Case Management and discharge planning. [Preferred]

Pay Range:
