What You’ll Do: Support residents and families through admissions, transitions, and ongoing care planning Conduct psychosocial assessments and develop individualized care plans Advocate for resident needs and ensure emotional and social well-being Facilitate care conferences and communicate updates with families and interdisciplinary teams Assist with discharge planning and coordination of services Ensure compliance with all federal, state, and facility regulations Provide support during difficult conversations and life transitions with empathy and professionalism What We’re Looking For: Bachelor’s degree in Social Work or related field required Social Services Designee certification (or willingness to obtain) preferred Experience in long-term care, post-acute, or senior living preferred Strong communication and interpersonal skills Ability to navigate sensitive situations with compassion and professionalism Highly organized with strong follow-through Why Hillcrest: Collaborative, team-focused environment Opportunities for growth across a multi-site organization Supportive leadership and strong clinical partnerships The ability to truly impact lives every day At Hillcrest, our values guide everything we do:Integrity.
