RN/LSW Clinic Coordinator

We are seeking a compassionate and experienced RN/LSW to join our Home Health Clinic team! This role is responsible for supporting patients through clinical care coordination, assessments, education, advocacy, and resource management to ensure the highest quality of care and overall well-being. The ideal candidate is organized, detail-oriented, and passionate about providing exceptional patient-centered support in a collaborative and caring environment. Minimum Requirements  Must have a valid Florida Registered Nurse License.  Must hold current Licensed Social Worker (LSW) credentials Must possess knowledge of laws, regulations, and guidelines pertaining to Senior Living, Home Health.   Minimum one-year supervisory experience in a healthcare setting.    Knowledge Skills & Abilities  Must have good organizational skills.   Must be a detail-oriented person and able to perform duties with great accuracy daily.  Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence.   Must have good communication skills including speaking, writing, and listening.  Must have excellent computer skills.  Must be proficient with Microsoft Office programs with emphasis on Excel, Outlook, and Word, with the ability to learn and use proprietary software as required.
