Job Summary:
Provides a safe, stimulating environment for program participants. Formulates, carries out and updates individual participant care plans. Initiates activities to achieve goals of the care plans. Demonstrates competency in caring for older adults and the geriatric client.
Job Requirements:
Education and Work Experience:
- High School Education/GED or equivalent: Preferred
- Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
- Experience as a home care or hospital nurse's aide: Preferred
Essential Functions:
- Initiates activities to keep clients active at all times. Remains aware of whereabouts of clients at all times to avoid wandering, falls or injuries.
- Updates client care plans as required.
- Maintains attendance records and statistics.
- Orders and maintains supplies.
- Performs other job-related duties as assigned.
