Job Description
Transitions LifeCare is seeking a full-time Float LPN to provide routine, PRN, and crisis care for hospice patients. This role offers phone support to patients and families, coordinates with team members, and performs office duties. Schedule: Monday-Friday, 8:00 AM – 5:00 PM. Sign-on bonus up to $5,000 for qualified candidates.
