Case Manager (RN), Tyrone & State College Hospitals

AS THE REGISTERED NURSE (RN) CASE MANAGER, you will be responsible for hospital case management, utilization review, discharge planning, and integrating social service functions into patient care. The Case Manager will provide quality patient care via the development, implementation, and evaluation of individual patient care plans. The RN will be responsible for determining the medical necessity of admissions and extended stays to meet the hospital's objectives for assuring high quality patient care with effective and efficient utilization of available health facilities and services. You will also make any and all arrangements for patients requiring help and/or services upon discharge from the hospital while working with community agents to coordinate patient care.

QUALIFICATIONS:

• Current Pennsylvania Registered Nursing license required
• Bachelor's degree in Nursing required 
• Travel between Penn Highlands Tyrone and State College Hospitals required
• At least 5 years of previous case management experience preferred
• Ability to develop policies and procedures, as well as evaluate clinical performances
• Must be able to function independently and have flexibility, personal integrity, and good communication skills with other healthcare team members
• Ability to seek out new methods and principles, then willing to incorporate them into existing practices
