Location: Milton Inpatient Unit at 597 Randolph Ave Milton, MA 02186

Schedule: Varied; 1 week/month, 1 weekend/month

Be the Best Hospice Inpatient Visit Physician You Can Be 

If you meet these qualifications, we would love to meet you: 

  • Maintains an active license and DEA in the state of Massachusetts
  • (Preferred) Hospice or hospice visit experience

 

Job Requirements:

  • Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families
  • Participate in prognosis evaluation and certification/recertification
  • Sign death certificates as needed
  • Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team
  • Ensure proper documentation of eligibility for inpatient care
  • Provide education and training to interdisciplinary team and new staff
  • Provide after-hours call coverage on days with inpatient visits
