Job Description
The Peer Health Navigator engages and connects members served to mental health, primary care, substance use, and specialty health care services; helps them develop personal health and wellness goals; educates them on how to advocate for themselves and effectively communicate their needs; and supports them in overcoming barriers and building resilience. The Peer Health Navigator supports members served in maintaining a follow-up care plan and developing other self-management skills through a “Modeling – navigator performs task; member served observes, Coaching – member served performs task; navigator coaches, Fading – member served self-directs and manages own healthcare; navigator assist as needed” process.
