Position Summary:

Responsible for placement, scheduling, and attendance confirmation of service for our clients. Main point of contact for both referral sources and providers

Key Responsibilities

• Placement and management of therapy, diagnostic and advanced services referrals according to parameters set forth by management to meet the client specific needs.

• Schedule patient with provider location, enter patient information into EMR system, send all necessary paperwork to the provider.

• Communicate with referral source the patient�s visit date, location, and time.

• Confirm patient attendance, provide update to referral source, along with any subsequent appointments. Update EMR system, collect records for electronic chart and send copies to referral source.

Required Qualifications

• High School Diploma or equivalent required
• Ability to type with minimal errors
• Strong proficiency in Microsoft Excel
• Excellent organizational, time-management, and multitasking skills

Work Environment & Physical Requirements

• Frequent standing, walking, sitting, bending, and computer use
• Ability to occasionally lift up to 30�50 pounds
