Lead Community Health Worker

Loyola Medicine, a member of Trinity Health, is a nationally ranked academic, quaternary care system based in Chicago's western suburbs. Loyola Medicine Transport is based at Loyola University Medical Center and is part of a three-hospital system including Loyola University Medical Center, Gottlieb Memorial Hospital, and MacNeal Hospital.   

A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served. Under general supervision, provides day-to-day guidance to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.

What you will do: 

  • Demonstrates Trinity Health’s Mission, Vision, and Values in all behaviors, decisions, and practices.
  • Supports daily operations of the CHW team; updates manager on responsibilities, performance, productivity, and resource needs.
  • Assigns and monitors incoming referrals across CHWs.
  • Ensures adherence to protocols (e.g., patient encounters, documentation) with accuracy and timeliness.
  • Manages a limited caseload: provides basic health education, connects individuals to services, and acts as a liaison between patients and providers.
  • Leads team huddles and conducts regular 1:1 check-ins to provide feedback and support development.
  • Promotes CHW services internally and in the community through events, meetings, and outreach activities.
  • Collaborates with supervisors and clinical teams to develop and refine CHW engagement and documentation protocols.
  • Supports program and service improvements to enhance care quality and outcomes.
  • Serves as a mentor and coach to CHWs.
  • Performs other duties as assigned.
  • Maintains compliance with all applicable laws, regulations, and organizational policies, upholding ethical and professional standards.

What you will need for this job: 

  • CHW experience required; must obtain Trinity Health (or equivalent) CHW certification within 3 months of hire.
  • High school diploma or GED with 3–5 years of community involvement experience.
  • Strong communication skills: able to read, write, interview patients, document accurately, and present information to individuals and small groups.
  • Ability to effectively communicate and collaborate with medical and social service professionals.
  • Oral communication and hearing ability required; bilingual skills preferred, with medical interpreter certification a plus.
  • Detail-oriented with ability to proofread and ensure document accuracy.
  • Meets all applicable state/local CHW requirements.
  • Basic Life Support (BLS) certification preferred.
  • Comfortable working in diverse, inclusive, and collaborative environments.
  • Demonstrates integrity, compassion, and the ability to inspire and motivate others in alignment with Trinity Health values.

Physical & Work Requirements:

  • Able to prioritize, multitask, and adapt in a fast-paced, sometimes stressful environment.
  • Ability to travel to patients’ homes as needed.
  • Complies with Trinity Health policies and procedures.

Compensation:  

Pay Range: $20.02 - $31.03 per hour  

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
