Growing together.As the Per Diem Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.Primary Responsibilities:Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of careInstructs the patients family in the care of the patient and maintenance of a healthy environment for the patientObserves, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition dailyAbility to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Required Qualifications:Current and unrestricted LPN licensure in state of practiceCurrent Driver's License, vehicle insurance, and access to a dependable vehicle or public transportationState Specific Requirements:LACurrent LPN Licensure in LA without restrictions.1+ years of experience working as an LPNPreferred Qualifications:Current CPR certification or ability to complete within 90 days of hire Home care experienceAbility to work flexible hours as required to meet identified patient needsAble to work independentlyGood communication, writing, and organizational skills
