Southwestern Behavioral Healthcare is currently seeking a full-time Care Coordinator to add to our team of professionals. This position will provide case management services and skills training/development services to adults in outpatient addictions services

WHAT IS THIS POSITION RESPONSIBLE FOR?
- Coordinate services primarily for adults seeking substance use disorder and co-occurring care from Southwestern.
- Assist with removing barriers to accessing services for clients and families.
- Conduct pre-intakes and assist with caseload management in order to reduce administrative burden for therapists and prescribers.
- Provide office and community-based skills training and development including parenting skills.
- Provide case management services for based on needs. Case management includes but it not limited to communicating with all parties involved in the treatment, identifying treatment needs, and facilitating all necessary referrals.
- Maintain appropriate records of services to reflect patient progress/needs; develop care plans and goals in conjunction with clients and referral sources.
- Maintain documents, collect data, and submit reports according to established time frames.
- Attend and participate in regular staff meetings.
- Participate in ongoing staff training and development activities as assigned.
- Provide emergency counseling and crisis intervention when necessary.
- Conduct consultation and education activities to community and professional groups as needed.
- Maintain contact with other service providers, community agencies, schools, and civic organizations to enhance community awareness and public relations.
MINIMUM QUALIFICATIONS:
- Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required.
- Experience working with adults with a substance use disorder is preferable.
- Possess the ability to assess emergency situations and react in an appropriate and therapeutic manner.
- Must have valid driver's license and qualify for Company's vehicle liability insurance.
- Candidates must pass required background checks including county/state checks, CPS check, sex offender registry check, and drug screen.
