What is an OhioRISE?

The Ohio Department of Medicaid (ODM) is committed to improving the health of Ohioans and strengthening communities and families through quality care. In 2020, ODM introduced a new vision for Ohio's Medicaid program - one that strengthens Ohio's future and ensures everyone has the chance to live life to its full potential.

OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs. Care Management Entities (CMEs) are vital to the success of the OhioRISE model and will each serve a separate catchment area – part of a county or multiple counties that make up their geographic footprint.

Job Title: OhioRISE CME Care Coordinator

Reports To: OhioRISE CME Supervisor

Direct Reports: None

FLSA Status: Exempt; Administrative (Para-professional)


 

Summary:

Applying the principles of Systems of Care, the OhioRise Moderate Care Coordinator is responsible for cultivating flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for OhioRISE members and in alignment with The Village Network’s Mission, Vision and Core Values. This role involves convening and facilitating Child and Family Teams (CFTs) developing individualized care plans, and ensuring collaboration among service providers, natural supports and community resources.

 

Essential Tasks, Duties, and Responsibilities:

  • Intake, Assessment and Planning: Initiate and maintain ongoing contact with children and families in the home and community setting; complete in-person assessments using assessment tools including the CANS; convene and facilitate in-person child and family team meetings; develop, implement and update the care plan; develop crisis safety plans; Request flex funds for services and items as approved by care plan; Provide in-person face to face and child and family team meetings at the required frequency and intensity required by tier level.
  • Service Coordination and Monitoring: Facilitate referrals and linkages to appropriate services and supports; Monitor the child and family centered care plan to endure services align with established goals; Coordinate treatment and educational efforts among providers and organizations; Ensure smooth transitions between facility-based and community-based care settings; Facilitate consistent and in person child and family team meetings; Provide additional member outreach to the member/guardian by phone at the frequency required by the tier level of the member.
  • Advocacy and Family Support: Advocate for clients’ needs including financial, housing, medical, legal, social, and environmental support; Assist clients and families in accessing public entitlements and maintaining benefits; Provide education and training to enhance families’ self-sufficiency and daily living skills; Encourage family-driven and youth guided planning and decision making.
  • Crisis Management: Collaborate with crisis response services to provide immediate and long term support; Ensure continuity of care and the safety of youth and families during crises; Provide after hours on-call support on rotating schedule which may include providing on-site and in-person support to members; Review and update crisis plans when appropriate or necessary
  • Documentation and Compliance: Complete progress notes and service documentation within specified timelines; Submit child and family-centered care plans to the OhioRISE plan within specified timelines; Maintain compliance with organizational policies and legal standards
  • Caseload: Maintain a caseload of 10 ICC or 20 MCC members
  • Training and Development: Participate in required in person orientation and ongoing training topics; Obtain and Maintain CANS certification; Attend staff meetings and individual supervisory conferences as scheduled; Participate in required continuing education utis to maintain QMHS or licensure status
  • Other duties as assigned.

 

Knowledge, Skills, and Abilities:

  • High School Diploma/GED with minimum of three years’ experience, or Associate/Bachelor’s degree with 2 years’ experience, or Master’s degree or higher with 1 year of experience providing community-based services and supports to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field.
  • May be licensed or an unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code.
  • Must complete the high-fidelity wraparound training program provided by the Cener of Excellence, an entity recognized by ODM.
  • Must successfully complete skill and competency-based training to provide Care Coordination
  • Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
  • Valid Ohio Driver’s License and maintains a driving record that allows the individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
  • Must travel for in-person and face-to-face requirements, including child and family team meetings and home visits. Must be able to visit member’s homes (may or may not be handicapped accessible).
  • Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
  • Maintain a HIPAA compliant office space

 

Physical Demands:

  • Occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
  • Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
  • Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
  • Must be able to lift 20 pounds independently
