Summary of Position:  This position provides patient-centered behavioral health services, in collaboration with the Patient Centered Medical Home (PCMH),  to patients at Barton Community Health Center, Barton’s Telehealth department, and other Barton clinics, with support from the medical director and office manager The position requires: assessment, conceptualization, treatment planning, self-direction, crisis intervention, using evidenced based therapies within professional scope of practice, adherence to W&I Codes and maintaining professional and ethical behavior in accordance with the National Association of Social Workers, with little to no supervision.
