Clinical Program Assistant - MOBILE HEALTH

Under the direction of Clinical Pharmacy Leadership and Clinical Pharmacist, the Clinical Program Assistant will perform assigned tasks necessary to support comprehensive clinical pharmacy services and disease management programs provided by the Clinical Pharmacists. The Clinical Program Assistant will be organizing and collecting clinical information for the clinical pharmacy appointments and medication reconciliation process.
