Overview:
Intake Coordinator/Engagement Specialist is responsible for completing intake assessments to all individuals who are seeking outpatient services and assist them with scheduling appointments with appropriate outpatient programs. Provides crisis intervention when needed. Makes appropriate referrals and consultations both within and outside the agency. Prepares and maintains client records. Tracks clients who are referred and scheduled for outpatient services to ensure the clients keep their appointments, coordinates with the providers to improve client engagement, and initiates discharges of clients who do not follow through with appointments.
ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES
- Conducts intake assessments for all new Medicaid and Uninsured clients seeking services. Determines needed services and appropriate level of care recommendation to outpatient program.
- Coordinates with outpatient supervisors to provide a warm hand off of clients for continuing services. When high risk individual is identified, communicate clients’ safety concerns and pertinent clients’ information to outpatient supervisors.
- Tracks clients who are referred and scheduled for outpatient services to ensure the client keeps their initial appointments, clients are engaged in the services they were referred to and initiates the discharge process of clients who do not follow through with initial appointments following Agency policies and procedures.
- Receives, tracks, screens, and engages clients referred to Psychosocial Rehabilitation groups. Coordinates with the BHT, Program Supervisor and TCM regarding referrals for Psychosocial Rehabilitation groups within the Outpatient Programs.
- Maintains all required and appropriate documentation for clinical records for assigned cases in accordance with Clinical Record Handbook specifications or Agency policies and procedures. Documentation must be finalized in Avatar within 48 hours from the time of intake appointment.
- Meets service productivity standard of at least 62.5% (or 5 out of 8 work hours) for the provision of assessment services during Same Day Access hours in accordance with applicable clinical/quality criteria which are supported by appropriate clinical record documentation which meet Clinical Record Handbook specifications, and Agency business/clinical ethics standards.
Education/Experience:
- Master’s degree with from an accredited university or college with a major in the field of counseling, social work, psychology, or a related human services field.
- Preferred one year of related professional experience in providing service to persons with psychiatric and behavioral illness. Professional experience working with persons with Substance Use disorders.
