Under the general direction of the rehabilitation manager, provides physical therapy services to patients in the outpatient following the plan of care established by a physical therapist.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Graduate of an approved associate degree program as a PTA

2. Current State PTA License

3. CPR certification     

EXPERIENCE:

1. Two (2) years of experience with a varied patient population

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1. Under the supervision of a physical therapist, provides physical therapy services,  and reassessing the patient’s status on each visit and ensuring quality of care as evidenced in the clinical record.

2. Coordinates patient care with other clinical team members ensuring optimal patient care and communication as evidenced in the clinical record.

3. Completes all required paperwork following policies and procedures to ensure that documentation reflects care provided and the outcomes achieved as evidenced in the clinical record.

4. Maintains and enhances personal knowledge as well as promoting professional standards

in physical therapy.

5. Instructs patient and caregiver in treatment programs, precautions and equipment use as necessary and documents patient/caregiver demonstration of instructions provided.

1. The National Institute for Occupational Safety and Health recommends minimizing the need for employees to manually lift patients, and limit safe patient handling to 35 pounds.  All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient’s profile and appropriate algorithms.

2. Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 40+lbs.

3. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
